Photograph, Present, and Publish

When contributors have their meeting with the Project Co-coordinator, Kathryn Evans, they will be asked to sign a release form – permission for Organization for Literacy in Lambton –  to photograph, present, and publish  their work for the Lambton Mosaic Project.

Contributors aged 18 years and older, can sign & date their own release form.

Contributors aged 17 and younger, must have their parent or guardian’s signature and date on the form.

The information on the actual form is as follows:

I hereby give permission for my Lambton Mosaic Project contribution to be photographed, published, and presented (if selected for the actual Lambton Map Mosaic), or published only (if not selected for the actual map).

Publishing covers an on-line e-book, as well as a physical photographic book of the contributions, and all entrants will be included.
